You Ask - I Answer

How can I make an appointment? 
Go to Contact and submit your request. You will be notified through email if your request is accepted or denied. 


What ages of clients do you see? 
Age 6 through 65

What forms need to be completed prior to my first appointment?
 You will receive an email with a secure link from our Electronic Health Record to access several forms. All the forms must be completed and sent back before you can schedule your first appointment.

What happens during psychiatric evaluation?
Angela Han, NP will gather a client's comprehensive medical/social/psychiatric history, build rapport with the client and families, formulate diagnoses, and discuss treatment options. 

What type of payment do you accept?
Payment by credit card will be processed during or after the appointment. Cash, check, Venmo, or Paypal are currently not accepted.

What is your cancellation policy?
Follow up appointment (30 min long) canceled or rescheduled within 24 hours before your appointment will be charged 50% of your appointment fee. Psychiatric evaluation appointment (60-90 min long) canceled or rescheduled within 48 hours before your appointment will be charged 50% of your appointment. Missed appointments will be charged 100% of the appointment fee.

Do you provide ADHD evaluation and medication treatment? Yes. Both pharmacological and nonpharmacological treatment options will be discussed but medication treatment will be an option only after ADHD diagnosis is confirmed, urine drug screen is completed, and  controlled substance history is reviewed.
